My Time Kids Academy prefers all immunizations to be current according to Texas Department of Health immunization schedule as well as, hearing and vision screenings for students 4 years of age and older. This documentation can be obtained through your child’s pediatrician. Affidavits of exemption must be submitted prior to enrollment for all students not current with immunizations and must be obtained through the State of Texas.
*Students with life-threatening medically diagnosed allergies must have an Allergy Action Plan on file (available in the front office). This form will need to accompany any medication or Epi-Pens. We must have all of this on or before your child’s first day.
